I solved the “viewing PDF problem” by clearing history on Chrome, Safari, and Firefox browsers, on Mac and Windows, when PDF fails to load!

In this example, PDF.js plugin is used to view a PDF from a file managed by the Download Monitor plugin, along with 2 PHP scripts I wrote for the Code Snippets plugin to retrieve a secure PDF file, then display incremental statistics: download count, and last date viewed.
